Endolymphatic stromal myosis: unique presentation and ultrastructural study

    Summary

Endolymphatic stromal myosis (ESM) can present as aggressive cervical polyps spreading to the peritoneum. Ultrastructural analysis suggests ESM is a low-grade stromal sarcoma, potentially treatable with progestagens.

    Area of Science:

    • Gynecologic Pathology
    • Reproductive Endocrinology
    • Surgical Oncology

    Background:

    • Endolymphatic stromal myosis (ESM) is a rare uterine tumor.
    • Its typical presentation and behavior are not fully elucidated.
    • Understanding ESM's origins is crucial for effective management.

    Observation:

    • A unique case of ESM presented as recurrent, frond-like cervical polyps.
    • The lesion exhibited aggressive behavior with peritoneal spread.
    • Histological and ultrastructural analysis revealed resemblance to proliferative endometrial stroma.

    Findings:

    • The tumor's ultrastructure supports the theory of ESM as a low-grade endometrial stromal sarcoma.
    • Aggressive clinical behavior was noted despite resemblance to benign stromal tissue.
    • Recurrent cervical polyposis is an atypical manifestation of ESM.

    Implications:

    • Ultrastructural findings may guide future therapeutic strategies for ESM.
    • Progestagen therapy is a potential treatment option for these unusual tumors.
    • This case expands the known clinical spectrum of endolymphatic stromal myosis.
