    Background:

    • Severe hip osteoarthritis necessitates advanced surgical interventions.
    • Traditional total hip replacement may not be ideal for all patient demographics.

    Observation:

    • A double cup hip surface replacement without cement was performed on 75 hips between 1972 and 1977.
    • Sixty-seven hips were followed for over 6 months post-procedure.

    Findings:

    • 86% of patients experienced significant pain relief after hip surface replacement.
    • Three reoperations were required, with successful management through further procedures.
    • No operative deaths, pulmonary embolism, thrombophlebitis, or deep infections were reported.

    Implications:

    • Hip surface replacement is a beneficial adjunctive procedure for younger patients (<60 years) with severe hip osteoarthritis.
    • This technique offers a viable alternative or precursor to more definitive surgeries like Charnley total hip replacement.
    • The study highlights the potential for improved patient outcomes and quality of life through innovative hip arthroplasty techniques.
