    Summary

    Type 1 diabetes patients who develop severe kidney disease (diabetic nephropathy) show no significant differences in metabolic control or other factors compared to those without nephropathy. Higher ketoacidosis rates were noted in patients who developed nephropathy.

    Background:

    • Diabetic nephropathy is a severe complication of Type 1 diabetes.
    • Identifying risk factors for diabetic nephropathy is crucial for patient outcomes.

    Purpose of the Study:

    • To compare clinical and metabolic parameters in Type 1 diabetes patients with and without severe diabetic nephropathy.
    • To investigate potential protective factors against kidney disease in Type 1 diabetes.

    Main Methods:

    • Comparison of 21 Type 1 diabetes patients with severe nephropathy (diagnosed after ~14.5 years) and 21 age/sex-matched Type 1 diabetes patients without nephropathy (diagnosed >32 years).
    • Analysis of body build, insulin needs, diabetes stability, heart rate, blood pressure, and metabolic control (using >1600 outpatient glucose measurements per patient).
    • Calculation of renal tubular glucose reabsorption capacity.

    • No significant differences were observed between groups in body build, insulin requirements, diabetes stability, heart rate, or blood pressure.
    • Metabolic control quality over the first 20 years did not differ significantly between groups.
    • A higher frequency of ketoacidosis was observed in the group that developed diabetic nephropathy.

    • Many individuals with Type 1 diabetes appear to possess protective mechanisms against kidney damage.
    • The specific factors conferring protection against diabetic nephropathy remain unidentified.
