Case report. Tentorial changes in sigmoid sinus thrombosis

H M Ingstrup, P S Jørgensen

    Journal of Computer Assisted Tomography
    |October 1, 1981
    PubMed
    Summary

Computed tomography (CT) revealed unique signs of cerebral sinovenous thrombosis, including dense tentorial incisura borders and enhancement. These findings suggest potential tentorial venous collateral pathways in cases of sigmoid sinus occlusion and stenosis.

    Area of Science:

    • Neurology
    • Radiology
    • Vascular Imaging

    Background:

    • Cerebral sinovenous thrombosis (CSVT) is a rare condition affecting venous drainage of the brain.
    • Accurate diagnosis is crucial for timely treatment and improved patient outcomes.
    • Computed tomography (CT) plays a role in diagnosing CSVT, but characteristic findings can be subtle.

    Observation:

    • A case of CSVT with confirmed right sigmoid sinus occlusion and left sigmoid sinus stenosis is presented.
    • Specific CT findings included abnormally dense borders of the tentorial incisura.
    • Postcontrast imaging demonstrated enhancement of the tentorium.

    Findings:

    • The observed CT features, specifically the dense tentorial incisura and enhancement, are associated with CSVT.

    Related Experiment Videos

  • These findings may indicate compensatory venous drainage mechanisms.
  • Tentorial venous collateral pathways are proposed as a potential explanation for the observed imaging characteristics.
  • Implications:

    • Understanding these CT findings can aid in the diagnosis of complex CSVT cases.
    • Recognition of tentorial venous collaterals may refine diagnostic criteria for CSVT.
    • Further research into venous collateral pathways in CSVT is warranted.
