    Summary

    Elderly patients with late onset schizophrenia may have undiagnosed hearing loss. Treating hearing impairment improved psychiatric symptoms and delusions in a geriatric patient.

    • A geriatric patient diagnosed with dementia presented with symptoms suggestive of paraphrenia, a late-onset schizophrenia.
    • Initial psychiatric assessment revealed intact cognition but delusional beliefs and disturbed behaviors.

    Observation:

    • The patient exhibited apparent hearing impairment during psychiatric evaluation.
    • Referral to an ENT clinic confirmed severe bilateral mixed hearing loss, likely present since childhood.

    Findings:

    • A hearing aid was prescribed and successfully fitted.
    • Following hearing aid use, the patient showed a significant reduction in disturbed behaviors and delusional thoughts.

    Implications:

    • This case highlights the potential link between untreated hearing loss and late-onset schizophrenia symptoms in the elderly.
    • Conjoint psychiatric and audiological assessment and treatment may improve outcomes for behaviorally disturbed geriatric patients.
