    Background:

    • Cervical disc lesions are a common cause of neck pain and neurological deficits.
    • Traditional surgical approaches often involve fusion techniques, which can lead to adjacent segment degeneration.

    Purpose of the Study:

    • To evaluate the efficacy of antero-lateral microsurgical discectomy without intercorporal graft for treating cervical disc lesions.
    • To compare the outcomes of this technique with traditional fusion methods.

    Main Methods:

    • A cohort of 25 patients underwent antero-lateral microsurgical discectomy.
    • Posterior osteophytes were removed, and the posterior longitudinal ligament was laterally resected.
    • Outcomes were assessed clinically, radiologically, and for working capacity over a 6-month to 4-year follow-up period.

    Main Results:

    • 84% of patients achieved excellent or good clinical and radiological outcomes.
    • Functional recovery and working capacity were significantly improved.
    • No significant differences were observed when compared to 51 patients treated with Cloward fusion techniques.

    Conclusions:

    • Antero-lateral microsurgical discectomy without intercorporal graft is an effective treatment for cervical disc lesions.
    • This technique offers comparable outcomes to fusion methods with potentially less invasiveness.
