    Background:

    • Bladder cancer management can be challenging for transurethrally unresectable tumors.
    • Radical cystectomy is a treatment option for advanced bladder cancer.

    Purpose of the Study:

    • To evaluate the efficacy of radical cystectomy and ileal conduit urinary diversion for Stage O or A bladder cancer.
    • To assess the impact of preoperative radiation therapy on outcomes.

    Main Methods:

    • One hundred nine patients with Stage O/A bladder cancer underwent radical cystectomy and ileal conduit urinary diversion.
    • Fifty-six patients received planned preoperative radiation therapy.
    • No lymphadenectomy was performed.

    Main Results:

    • The overall five-year survival rate was 76%.
    • Preoperative radiation eradicated all tumor (Po) in 36% of patients.
    • Irradiated patients had a lower local recurrence rate (3.5%) compared to non-irradiated patients (9.4%).
    • Urethral cancer was found in 9% of patients, not supporting previous hypotheses on multifocal tumors.

    Conclusions:

    • Radical cystectomy with ileal conduit diversion is effective for Stage O/A bladder cancer.
    • Preoperative radiation therapy improves local control and reduces recurrence in these patients.
    • The incidence of urethral cancer in this cohort did not correlate with tumor multifocality or prostatic urethra involvement.
