Delayed hypersensitivity to thimerosal in soft contact lens wearers

    Summary

    Soft contact lens wearers experiencing eye irritation and corneal issues may have a thimerosal hypersensitivity. Switching to preservative-free solutions resolved symptoms in most patients.

    Background:

    • Soft contact lens wear is common, but can be associated with ocular surface complications.
    • Thimerosal, a mercury-based preservative, is used in some contact lens solutions.

    Observation:

    • Patients presented with ocular redness, irritation, and transient corneal changes, including epithelial opacities and punctate keratopathy.
    • All patients used thimerosal-containing solutions for lens care.
    • A significant number reacted to ocular challenge with thimerosal lubricant and patch testing.

    Findings:

    • Thirty-one of 38 patients showed a hypersensitivity to thimerosal.
    • Symptoms resolved upon discontinuing thimerosal-containing products.
    • Using unit-dose, preservative-free saline with thermal disinfection allowed resumption of lens wear.

    Implications:

    • Thimerosal hypersensitivity is a likely cause of contact lens-related corneal complications.
    • Preservative-free contact lens care regimens can effectively manage these reactions.
    • Ophthalmologists should consider thimerosal allergy in patients with unexplained contact lens-related ocular surface disease.
