    Background:

    • Malignant hyperthermia (MH) is a rare, severe hypermetabolic crisis.
    • Triggered by volatile anesthetics and succinylcholine in susceptible individuals.
    • Equine MH susceptibility is linked to RYR1 gene mutations.

    Observation:

    • A 4-year-old Thoroughbred horse developed MH during halothane anesthesia and succinylcholine administration.
    • Clinical signs included fever, sweating, hyperventilation, tachycardia, and blood pressure fluctuations.
    • Biochemical changes revealed hypocalcemia, hyperkalemia, hyperphosphatemia, myoglobinuria, and elevated muscle enzymes.

    Findings:

    • Aggressive cooling (ice, cold solutions) and supportive care were initiated.
    • Anesthetic agents were discontinued, and oxygen was administered.
    • Treatment included IV fluids, sodium bicarbonate, xylazine, and dantrolene, with further dantrolene and supportive care post-anesthesia.
    • Muscle biopsy confirmed MH with exaggerated halothane and caffeine contracture responses.

    Implications:

    • Early recognition and aggressive management are vital for MH survival in horses.
    • Dantrolene sodium is a critical therapeutic agent for equine MH.
    • This case highlights the importance of anesthetic monitoring and preparedness for adverse reactions in equine practice.
