    Malignant transformation of solitary exostosis is undetermined. Multiple exostoses showed a 13% malignant transformation rate, though the true incidence of exostosis cancer may be higher.

    Background:

    • Exostosis, also known as osteochondroma, is a benign bone tumor.
    • Malignant transformation into chondrosarcoma is a rare but serious complication.
    • Understanding the incidence of malignant transformation is crucial for patient management.

    Purpose of the Study:

    • To investigate the incidence of malignant transformation in solitary and multiple exostoses.
    • To examine the relationship between exostosis and peripheral chondrosarcoma.

    Main Methods:

    • Retrospective analysis of 408 exostosis cases (318 single, 90 multiple).
    • Review of 73 peripheral chondrosarcoma cases from the Rizzoli Institute Tumour Centre.
    • Separate study of single and multiple exostosis cases.

    Main Results:

    • The incidence of malignant transformation in solitary exostosis could not be determined.
    • A 13% incidence of malignant transformation was observed in the studied cases of multiple exostoses.
