    Background:

    • Diffuse osteopenia is not a commonly recognized radiological sign of skeletal sarcoidosis.
    • The pathological basis of skeletal abnormalities in sarcoidosis remains poorly understood.

    Observation:

    • A patient with sarcoidosis presented with diffuse osteopenia, hypercalcemia, and hypercalciuria.
    • The patient received intermittent treatment with corticosteroids and thiazides.
    • Histomorphometric analysis of a bone biopsy was performed.

    Findings:

    • Peritrabecular granulomas were observed in the bone biopsy.
    • Histological features indicated accelerated bone remodeling: excess osteoid, increased osteoclast and osteoblast numbers, increased osteoclastic resorbing surfaces, and elevated bone formation rates.

    Implications:

    • Peritrabecular granulomas in skeletal sarcoidosis can drive rapid bone turnover.
    • This accelerated remodeling may lead to diffuse osteopenia and contribute to mineral metabolism disturbances.
