    Summary

    Splenomegaly in sarcoidosis patients indicates more widespread extrathoracic disease but not worse lung involvement. Corticosteroids are recommended only for greatly enlarged spleens unless other specific indications exist.

    Background:

    • Sarcoidosis is a multisystem inflammatory disease.
    • Splenomegaly is a known manifestation of sarcoidosis.

    Purpose of the Study:

    • To investigate the clinical characteristics and outcomes of sarcoidosis patients with splenomegaly.
    • To compare extrathoracic and pulmonary involvement in sarcoidosis patients with and without splenomegaly.
    • To evaluate the indications for corticosteroid therapy in sarcoidosis-associated splenomegaly.

    Main Methods:

    • Retrospective review of clinical records of 32 sarcoidosis patients with splenomegaly.
    • Comparison with a matched control group without splenomegaly.
    • Assessment of extrathoracic and pulmonary sarcoidosis extent, and presence of hypersplenism and abdominal symptoms.

    Main Results:

    • Patients with splenomegaly exhibited more extensive extrathoracic sarcoidosis compared to controls.
    • No significant difference in pulmonary involvement was observed between groups.
    • Hypersplenism and abdominal symptoms were present in 20% and 16% of splenomegaly patients, respectively, primarily in those with greatly enlarged spleens.

    Conclusions:

    • Splenomegaly in sarcoidosis is associated with more extensive extrathoracic disease.
    • Corticosteroid treatment for splenomegaly should be reserved for significantly enlarged spleens, barring other specific indications.
    • Management decisions regarding splenomegaly in sarcoidosis should consider spleen size and associated complications.
