    Bilateral optic nerve sheath meningiomas caused progressive vision loss in a patient despite normal initial imaging. Multifocal tumors were discovered after a 15-year decline in vision.

    Background:

    • Optic nerve sheath meningiomas (ONSMs) are rare tumors arising from glial cells within the optic nerve sheath.
    • They can present with gradual vision loss, optic disc swelling, and visual field defects.

    Observation:

    • A 35-year-old male presented with incidental bilateral optic disc swelling.
    • Over 15 years, he experienced progressive, irreversible vision loss.
    • Opticocillary shunt vessels were noted on both optic discs, with normal neuroradiologic findings initially.

    Findings:

    • Surgical exploration via craniotomy revealed multifocal bilateral optic nerve sheath meningiomas.
    • The tumors appeared to have arisen from multiple sites along the optic nerves.
