    Open chest massage after mitral valve replacement with porcine valves can cause left ventricle laceration. This rare complication highlights the need for caution during cardiac resuscitation in these patients.

    Background:

    Observation:

    • A case of cardiac resuscitation is reported after mitral valve replacement with a stented porcine valve.
    • The patient underwent both closed and open chest cardiac massage.

    Findings:

    • The patient died following prolonged open chest massage.
    • Autopsy revealed laceration of the left ventricle caused by the prosthetic valve stents.

    Implications:

    • This case highlights a previously undescribed complication of open chest massage in patients with stented porcine mitral valves.
    • Physicians must exercise extreme caution during cardiac resuscitation in the vicinity of these prosthetic valves.
