    Background:

    • Accurate determination of human plasma oxalate concentration is crucial for clinical diagnostics.
    • Previous enzymatic assay methods may overestimate oxalate levels due to in vitro glyoxalate conversion.

    Purpose of the Study:

    • To develop and validate a more accurate method for measuring human plasma oxalate concentration.
    • To investigate and mitigate the in vitro conversion of glyoxalate to oxalate in blood samples.

    Main Methods:

    • Deproteinised plasma was used for an enzymatic assay.
    • Inhibitors of glyoxalate oxidation (boric acid, DL-beta-phenyllactic acid, allopurinol) were added to whole blood samples prior to analysis.
    • Plasma oxalate levels were measured using the same enzymatic technique before and after inhibitor addition.

    Main Results:

    • Initial assays on deproteinised plasma from normal subjects showed a mean oxalate concentration of 7.78 mumol/l (range 1.1–16.0 mumol/l).
    • The conversion of glyoxalate to oxalate in whole blood in vitro was confirmed.
    • After adding inhibitors to fresh blood samples, the mean plasma oxalate concentration decreased significantly to 2.26 mumol/l (range 0–5.4 mumol/l).

    Conclusions:

    • Normal blood spontaneously generates oxalate upon standing.
    • The addition of specific inhibitors effectively prevents in vitro oxalate generation.
    • The revised method provides a more accurate measurement of basal human plasma oxalate concentration, suggesting previous high values were fallacious.
