    Summary

    This study presents the first fatal case of cemented tungsten carbide pneumoconiosis. Cobalt toxicity is suspected in pathogenesis, alongside individual susceptibility, in this severe lung fibrosis.

    Background:

    • Cemented tungsten carbide (WC-Co) is widely used in industrial applications.
    • Occupational exposure to WC-Co dust can lead to respiratory diseases.

    Observation:

    • A 28-year-old woman developed interstitial pulmonary fibrosis after four years of grinding WC-Co.
    • Symptoms progressed despite corticosteroid therapy, leading to fatality five years after onset.

    Findings:

    • Autopsy revealed nonspecific interstitial pneumonitis and marked lung fibrosis.
    • Electron probe microanalysis detected tungsten and WC constituents in lung tissue, but not cobalt.
    • Elevated tungsten levels were found in the lung, and cobalt levels were tenfold higher in bone.

    Implications:

    • Cobalt toxicity is a primary suspect in the pathogenesis of WC-Co pneumoconiosis.
    • Individual susceptibility may play a significant role in disease development.
    • This case highlights the severe occupational health risks associated with cemented tungsten carbide exposure.
