Lightning injuries: prognostic signs for death

M A Cooper

    Annals of Emergency Medicine

    Summary

Lightning strike victims with leg burns, cranial burns, or cardiopulmonary arrest face a poor prognosis. Immediate resuscitation is crucial for those experiencing loss of consciousness to improve survival rates.

    Area of Science:

    • Medical Research
    • Trauma Care
    • Lightning Injury

    Background:

    • Lightning strikes are a significant cause of injury and mortality worldwide.
    • Understanding prognostic factors is crucial for effective trauma management.
    • Previous studies have explored lightning injury outcomes, but prognostic indicators require further clarification.

    Purpose of the Study:

    • To identify key prognostic indicators for mortality in severe lightning-injured patients.
    • To analyze the relationship between specific injury patterns and patient outcomes.
    • To provide evidence-based recommendations for resuscitation efforts.

    Main Methods:

    • A chi-square analysis was performed on 66 lightning injury cases (8 author's, 58 literature).
    • Data included patient demographics, injury types, and clinical outcomes.
    • A review of lightning physics, pathophysiology, and injury mechanisms was conducted.

    Main Results:

    • Overall mortality rate was 30% in the studied cases.
    • Leg burns (30% mortality), cranial burns (37% mortality), and cardiopulmonary arrest (76% mortality) were associated with poor prognosis.
    • Age, sex, and trunk/arm burns were not significant prognostic factors.
    • Permanent sequelae occurred in 74% of survivors.

    Conclusions:

    • Cardiopulmonary arrest is the strongest predictor of mortality following severe lightning injury.
    • Prompt resuscitation for victims with loss of consciousness is critical.
    • Specific burn locations (legs, cranium) indicate a higher risk of death.
