    Background:

    • Multiple myeloma is a hematologic malignancy characterized by plasma cell proliferation.
    • Bone disease and hypercalcemia are common complications of multiple myeloma.
    • Flail chest is a severe consequence of chest wall trauma, but can also arise from underlying pathology.

    Observation:

    • Two patients with multiple myeloma presented with pathologic flail chest.
    • Both patients exhibited severe hypercalcemia and diffuse bone disease.
    • Pulmonary complications like atelectasis and edema preceded or exacerbated the flail chest.

    Findings:

    • Treatment with radiotherapy and chemotherapy was initiated for both patients.
    • Delayed chest wall stabilization with positive airway pressure in one patient led to worsening flail chest and respiratory failure.
    • Prolonged chest wall stabilization in the second patient, concurrent with chemotherapy, resulted in flail chest resolution.

    Implications:

    • Pathologic flail chest is a rare but serious complication of multiple myeloma, particularly with severe hypercalcemia.
    • Early and sustained chest wall stabilization is critical for managing flail chest in these patients.
    • This case highlights the importance of integrated management strategies combining oncologic treatment with respiratory support.
