    Summary

    Patients with extensive ulcerative colitis face a significantly increased risk of developing colorectal cancer. This study highlights the importance of long-term monitoring for these individuals to detect cancer early.

    Background:

    • Ulcerative colitis is a chronic inflammatory bowel disease.
    • Patients with extensive disease are at higher risk for colorectal cancer.

    Purpose of the Study:

    • To estimate the cumulative risk of colorectal cancer in patients with extensive ulcerative colitis.
    • To identify risk factors and incidence rates for colorectal cancer in this population.

    Main Methods:

    • A cohort of 234 patients with extensive ulcerative colitis in Göteborg, Sweden, diagnosed between 1951 and 1974.
    • Follow-up until December 1975 with a mean observation time of 8.5 years.
    • Comparison of observed cancer cases with expected numbers in a matched reference group.

    Main Results:

    • Fifteen patients developed colorectal cancer during the follow-up period.
    • The cumulative incidence of colorectal cancer 25 years after colitis onset was 34%.
    • Patients diagnosed before age 25 had a 43% cumulative incidence at 25 years.

    Conclusions:

    • Extensive ulcerative colitis confers a substantially elevated risk for colorectal cancer.
    • Early-onset colitis (<25 years) is associated with a particularly high cumulative cancer risk.
    • Regular surveillance is essential for patients with extensive ulcerative colitis to mitigate cancer risk.
