    Background:

    • Parkinson's disease is a neurodegenerative disorder affecting motor control.
    • Action tremor is a common symptom in Parkinson's disease patients.
    • The underlying mechanisms of Parkinsonian tremor are not fully understood.

    Purpose of the Study:

    • To investigate the electromyographic activity of the biceps muscle in Parkinsonian patients during specific movements.
    • To compare muscle activity patterns between Parkinsonian patients and healthy controls.
    • To explore the role of internal feedback circuits in the generation of Parkinsonian action tremor.

    Main Methods:

    • Electromyography (EMG) was used to record biceps muscle activity.
    • Participants performed rapidly alternating pronation-supination movements (RAM).
    • Single pronation or supination movements were triggered by visual or kinesthetic cues.

    Main Results:

    • Parkinsonian patients predominantly showed rhythmic biceps EMG activity during supination phases of RAM.
    • Normal subjects displayed continuous biceps EMG activity during the same movements.
    • Rhythmic activity occurred in Parkinsonian patients even before limb movement initiation, triggered by voluntary effort.

    Conclusions:

    • Voluntarily initiated activity in Parkinsonian patients may lead to oscillations in sensorimotor feedback loops.
    • These oscillations, involving cortico-spinal pathways, are hypothesized to cause action tremor.
    • The findings provide insights into the neural basis of Parkinsonian tremor.
