    Summary

    Calcified thrombus in extrahepatic portal vein obstruction can cause obstructive jaundice by compressing the common bile duct. This finding aids in diagnosing bile duct compression in such cases.

    Background:

    • Extrahepatic portal vein obstruction (EHPVO) is a rare condition.
    • Obstructive jaundice can be a presenting symptom of EHPVO.

    Observation:

    • Two patients with EHPVO presented with obstructive jaundice.
    • Calcification within the portal vein thrombus was identified in both patients.

    Findings:

    • Percutaneous cholangiography and endoscopic retrograde cholangiopancreatography (ERCP) revealed common bile duct compression.
    • The compression was attributed to a calcified cavernoma, a vascular malformation, within the portal vein thrombus.

    Implications:

    • Calcification within EHPVO thrombi can lead to bile duct compression and jaundice.
    • Radiological and endoscopic investigations are crucial for diagnosing the cause of obstructive jaundice in EHPVO.
    • Understanding this mechanism can improve diagnostic accuracy and patient management.
