    Background:

    • Spinal cord injury (SCI) patients frequently present with clinical signs suggestive of hypothyroidism.
    • Routine thyroid function testing is necessary to accurately diagnose or exclude thyroid hypofunction in this population.

    Observation:

    • Two cases of hypothyroidism, initially uncertain based on clinical presentation alone, were identified through low serum thyroxine screening tests.
    • These cases were subsequently confirmed by elevated serum thyroid-stimulating hormone (TSH) levels.

    Findings:

    • Low serum triiodothyronine (T3) levels are common in SCI patients who appear euthyroid.
    • Low serum T3 levels in tetraplegic patients do not necessarily indicate clinical hypothyroidism.

    Implications:

    • Highlights the importance of comprehensive thyroid function testing in spinal cord injury patients.
    • Suggests that low T3 levels in euthyroid tetraplegic individuals may be a physiological adaptation rather than indicative of hypothyroidism.
