    Background:

    • Expanded polytetrafluoroethylene (PTFE) is a promising material for arterial and venous substitutes.
    • PTFE grafts typically exhibit excellent tissue incorporation and neointimal lining formation.

    Observation:

    • Three recovered PTFE grafts demonstrated atheromatous changes within the neointimal lining.
    • These changes were distinct from suture-line neointimal hyperplasia based on location, imaging, and staining.
    • Two grafts were used for chronic hemodialysis access, suggesting potential contributing factors.

    Findings:

    • Pathological examination revealed atheromatous changes in the neointima of PTFE vascular grafts.
    • The observed changes were differentiated from typical neointimal hyperplasia.
    • Potential contributing factors in hemodialysis patients include chronic renal failure and repeated needle punctures.

    Implications:

    • The findings raise questions about the long-term healing properties of expanded PTFE.
    • Further investigation is required to determine if expanded PTFE possesses atherogenic potential.
    • This study highlights the need to evaluate PTFE's suitability for vascular reconstruction, especially in high-risk patients.
