Laboratory evaluation of heparin therapy

Plasma heparin levels measured using several different methods showed a wide variation in results. In particular an anti-factor Xa clotting technique gave very much higher results than thrombin clotting time and partial thromboplastin time techniques. This was less marked when a synthetic chromogenic substrate assay was used. Possible reasons for these observations and their relevance in the selection of therapeutic ranges are discussed.
