    Summary

    Multicystic ovaries and early puberty in girls with hypothyroidism may resolve with thyroid hormone replacement. Pelvic sonography may reveal these ovarian changes more frequently in hypothyroid children.

    Background:

    • Primary hypothyroidism in children can present with varied symptoms.
    • Sexual precocity is an uncommon but documented complication of hypothyroidism.
    • Multicystic ovarian changes require further investigation in pediatric cases.

    Observation:

    • Pelvic sonography identified multicystic ovaries in four girls with long-standing primary hypothyroidism.
    • Three of these girls also exhibited modest sexual precocity.
    • Ovarian cysts were not clinically apparent in most patients.

    Findings:

    • The multicystic ovarian lesions regressed rapidly following initiation of thyroid hormone replacement therapy.
    • Elevated baseline plasma luteinizing hormone (LH) levels were observed in three of four girls, contrasting with low or normal urinary LH output.
    • Follicle-stimulating hormone levels remained normal, and gonadotropin responses to luteinizing hormone-releasing hormone infusion were variable.

    Implications:

    • Pelvic sonography may be a valuable tool for detecting multicystic ovaries in hypothyroid girls.
    • Thyroid hormone replacement appears effective in resolving these ovarian abnormalities.
    • The hormonal mechanisms underlying the cystic ovarian changes and associated sexual precocity in hypothyroidism warrant further research.
