    Background:

    • Meniere's disease is a disorder of the inner ear.
    • It is characterized by episodes of vertigo, tinnitus, and hearing loss.

    Purpose of the Study:

    • To investigate the pathological changes in the cochlear nerve of patients with advanced Meniere's disease.

    Main Methods:

    • Light and electron microscopy were used to examine the cochlear nerves of 11 patients.
    • Nerve fiber density was quantified in cross-sections.

    Main Results:

    • Light microscopy revealed generally normal cochlear nerves.
    • Electron microscopy indicated signs of primary axonal degeneration or segmental demyelination.

    Conclusions:

    • The findings suggest that cochlear nerve degeneration may play a role in the pathophysiology of advanced Meniere's disease.
    • Further research is needed to elucidate the exact mechanisms.
