A comparison between midline and mediolateral episiotomies

P M Coats, K K Chan, M Wilkins

British Journal of Obstetrics and Gynaecology
    |May 1, 1980
    PubMed
    Summary

Midline episiotomy resulted in less noticeable scarring and earlier return to intercourse compared to mediolateral episiotomy. However, midline procedures were associated with a higher incidence of anal sphincter injury.

    Area of Science:

    • Obstetrics and Gynecology
    • Surgical Procedures
    • Women's Health

    Background:

    • Episiotomy is a surgical procedure during childbirth.
    • Midline and mediolateral episiotomies are two common techniques with different associated risks and benefits.
    • Understanding the long-term consequences is crucial for clinical decision-making.

    Purpose of the Study:

    • To compare the outcomes of midline versus mediolateral episiotomies in primiparous patients.
    • To evaluate pain, anal sphincter injury, scarring, and sexual function after different episiotomy types.

    Main Methods:

    • A randomized prospective study involving 407 primiparous women.
    • Comparison of pain estimates, anal sphincter injury rates, scarring appearance, and time to resuming intercourse between the two episiotomy groups.

    Main Results:

    • Patient-reported pain levels were similar between midline and mediolateral episiotomy groups.
    • Midline episiotomies showed a significantly higher rate of anal sphincter injury.
    • No rectovaginal fistulae were observed in either group.
    • Less noticeable scarring and earlier resumption of intercourse were reported following midline episiotomy.

    Conclusions:

    • While midline episiotomy may offer benefits in terms of scarring and sexual recovery, the increased risk of anal sphincter injury requires careful consideration.
    • The choice between midline and mediolateral episiotomy should be individualized based on patient factors and surgical expertise.
