Sleep apnoea in acromegaly

W H Perks, P M Horrocks, R A Cooper

    British Medical Journal
    |March 29, 1980
    PubMed
    Summary

Sleep apnoea is common in acromegaly, affecting half of patients studied. High growth hormone levels correlate with sleep apnoea, indicating a need for monitoring in acromegaly patients.

    Area of Science:

    • Endocrinology
    • Sleep Medicine
    • Pulmonology

    Background:

    • Acromegaly, a condition caused by excess growth hormone, is associated with various health complications.
    • Sleep-related breathing disorders, including sleep apnoea, have been anecdotally reported in acromegaly patients.

    Purpose of the Study:

    • To investigate the prevalence and clinical significance of sleep apnoea in patients with acromegaly.
    • To explore the relationship between growth hormone levels and sleep apnoea in this cohort.

    Main Methods:

    • Prospective study involving 11 acromegaly patients.
    • Clinical assessment for daytime somnolence and excessive snoring.
    • Polysomnography to diagnose sleep apnoea.
    • Flow-loop analysis for upper airway obstruction.
    • Measurement of serum growth hormone concentrations.

    Main Results:

    • Five out of 11 acromegaly patients experienced daytime somnolence or excessive snoring.
    • All five symptomatic patients had sleep apnoea episodes; three met criteria for sleep apnoea syndrome.
    • Patients with sleep apnoea had significantly higher growth hormone levels (p < 0.025).
    • Upper airway obstruction was observed in two patients.
    • Cardiomegaly was present in two patients with sleep apnoea syndrome.

    Conclusions:

    • Sleep apnoea is a common and clinically significant comorbidity in acromegaly.
    • Elevated growth hormone levels are associated with an increased risk of sleep apnoea.
    • Polygraphic nocturnal monitoring is recommended for the comprehensive assessment of acromegaly patients.
