    Metastasis of breast cancer to the bladder is rare but may be more common than suspected. This urinary bladder complication, marked by hematuria, can occur years after initial breast cancer diagnosis.

    • Primary breast carcinoma is a common malignancy in women.

    Observation:

    • This report details four cases of primary breast carcinoma with secondary metastasis to the urinary bladder.
    • Hematuria was a presenting symptom in three of the four patients.
    • Two patients had a history of steroidal drug use, potentially linked to metastasis.

    Findings:

    • Metastasis to the bladder occurred at least 32 months post-breast cancer diagnosis.
    • In two cases, bladder metastasis was identified shortly before patient demise.
    • Steroidal drug use was noted in a subset of patients, suggesting a possible contributing factor.

    • The findings suggest that bladder metastasis from breast cancer may be underdiagnosed.
    • Clinicians should consider urinary tract metastasis in breast cancer patients presenting with hematuria.
