Phenytoin hypersensitivity presenting as postoperative fever

Background:

Intracranial surgery carries a risk of serious complications, including infection.
Phenytoin is commonly administered postoperatively to prevent seizures.

Observation:

Two cases are presented where postoperative fever was the initial sign of phenytoin sensitivity syndrome.
The full syndrome developed later, highlighting that not all initial signs may be present.
Phenytoin hypersensitivity can manifest atypically.

Findings:

Phenytoin sensitivity syndrome can present initially with only postoperative fever.
The syndrome's components may not all be immediately apparent.
