    Background:

    • Postoperative intracranial changes can present diagnostic challenges.
    • Ring enhancement on computed tomography (CT) is a common finding after neurosurgery.
    • Differentiating benign postoperative enhancement from serious intracranial pathology is critical.

    Observation:

    • Nine patients with suspected postoperative intracranial infection were evaluated.
    • All patients exhibited ring-enhancing lesions on CT scans.
    • The appearance of enhancement mimicked both residual neoplasia and cerebral abscess.

    Findings:

    • Serial and sequential-delayed CT imaging proved effective in differentiating enhancement patterns.
    • This imaging approach distinguished normal postoperative changes from active infections.
    • The findings suggest a non-invasive method to resolve diagnostic ambiguity.

    Implications:

    • Serial and delayed CT can prevent misdiagnosis of intracranial infections.
    • Unnecessary surgical re-exploration may be avoided through accurate imaging interpretation.
    • Improved patient management and outcomes are possible with advanced CT techniques.
