Phospholine iodide toxicity and Jones' tubes

J R Wood, R L Anderson, J J Edwards

    Ophthalmology
    |April 1, 1980

    Summary

Topical glaucoma medication toxicity can occur after eye surgery. Discontinuation of echothiophate iodide drops resolved severe systemic symptoms in a patient who underwent dacryocystorhinostomy.

    Area of Science:

    • Ophthalmology
    • Pharmacology
    • Gastroenterology

    Background:

    • Glaucoma treatment often involves topical medications.
    • Canalicular stenosis can necessitate surgical intervention like conjunctivodacryocystorhinostomy with Jones tubes.

    Observation:

    • A 72-year-old man developed severe diarrhea, fatigue, weight loss, and prostration post-surgery.
    • Symptoms emerged shortly after bilateral conjunctivodacryocystorhinostomy for canalicular stenosis.
    • These symptoms were previously unexplained and life-threatening.

    Findings:

    • Discontinuation of echothiophate iodide (phospholine iodide) drops led to rapid symptom resolution.
    • This suggests enhanced systemic absorption of topical medications after lacrimal surgery.
    • Drug toxicity is an unreported complication of conjunctivodacryocystorhinostomy.

    Implications:

    • Ophthalmologists must consider systemic drug toxicity after lacrimal surgery.
    • Enhanced absorption applies to long-acting anticholinesterases, sympathomimetics, and cycloplegics.
    • Awareness is crucial for managing potential adverse effects in patients.
