Toxic nephropathy during continuous rifampin therapy

    Summary

    Rifampin can cause acute kidney injury, specifically acute interstitial nephritis, even with daily use. Steroid therapy may help manage this rifampin-induced nephropathy.

    Background:

    • Rifampin is a crucial antibiotic for tuberculosis treatment.
    • Concomitant steroid use is common in patients requiring rifampin.
    • Acute kidney injury (AKI) is a potential adverse effect of various medications.

    Observation:

    • A 54-year-old male patient developed nonoliguric acute renal failure during daily rifampin therapy.
    • The patient was also receiving steroid medication concurrently.
    • Renal biopsy revealed acute interstitial nephritis with tubular dysfunction.

    Findings:

    • The patient exhibited impaired urine concentration, increased fractional excretion of sodium and uric acid, and glycosuria with normal blood glucose.
    • Renal abnormalities resolved rapidly after discontinuing rifampin.
    • Steroid therapy was gradually tapered during the resolution phase.

    Implications:

    • This case highlights that rifampin-induced nephropathy can manifest during continuous daily dosing regimens.
    • Early recognition and withdrawal of rifampin are key to managing this condition.
    • Steroid administration may favorably influence the outcome of rifampin-induced acute interstitial nephritis.
