A study of spontaneously aborted twins

J E Livingston, B J Poland

    Summary

Spontaneous twin abortions occur at a rate of 1 in 35, with a high incidence of abnormalities, especially in twin embryos. This suggests twinning, particularly monozygotic twinning, is more common than previously believed.

    Area of Science:

    • Reproductive Biology
    • Developmental Biology
    • Genetics

    Background:

    • Twinning is a significant factor in human reproduction.
    • Understanding twinning rates and outcomes is crucial for reproductive health.
    • Previous data relied heavily on liveborn twin statistics.

    Purpose of the Study:

    • To determine the incidence of twinning in spontaneous abortions.
    • To analyze the zygosity and abnormality rates of spontaneously aborted twins.
    • To compare findings with data from liveborn twins.

    Main Methods:

    • Examination of 1,939 spontaneously aborted embryos and fetuses.
    • Identification and analysis of 53 twin pairs.
    • Zygosity determination for 37 twin pairs.
    • Assessment of abnormality rates in twin embryos and fetuses.

    Main Results:

    • A spontaneous abortion twinning rate of approximately 1 in 35 was observed.
    • The monozygotic to dizygotic twin ratio was 17.5:1.
    • Abnormality rates were high: 88% in twin embryos and 21% in twin fetuses.
    • Embryonic and fetal mortality was significantly higher in twins compared to singletons.

    Conclusions:

    • Twinning, especially monozygotic twinning, occurs more frequently than suggested by liveborn data.
    • Spontaneous abortion plays a critical role in the high mortality of twin gestations.
    • Early embryonic and fetal loss is a major factor in twin survival rates.
