Pseudohydronephrosis on bone scan due to crossed renal ectopia

    Summary

    A bone scan can mimic hydronephrosis and a nonfunctioning kidney in patients with crossed renal ectopia. This imaging finding necessitates further investigation to identify the ectopic kidney, even without obstruction.

    • Crossed renal ectopia is a rare congenital anomaly where both kidneys are located on the same side of the spine.

    • A patient presented with imaging findings suggestive of unilateral hydronephrosis and a contralateral absent or nonfunctioning kidney on bone scan.
    • Subsequent excretory urogram confirmed crossed renal ectopia without evidence of obstruction.

    Findings:

    • The bone scan's appearance mimicked hydronephrosis and renal agenesis.
    • The excretory urogram accurately diagnosed crossed renal ectopia.
