    Summary

    Acoustic reflex thresholds (ARTs) cannot reliably estimate hearing thresholds in adults with sensorineural hearing loss. While ARTs show some correlation with hearing loss up to 70 dB, they are inadequate for clinical or medico-legal use.

    Background:

    • Claims exist suggesting acoustic reflex thresholds (ARTs) can estimate hearing thresholds.
    • Previous research has not definitively established the accuracy of ARTs for predicting hearing loss.

    Purpose of the Study:

    • To statistically evaluate the relationship between puretone thresholds and ARTs in adults with sensorineural hearing loss.
    • To assess the predictive accuracy of ARTs for estimating hearing loss severity and audiogram features.

    Main Methods:

    • Analysis of a large dataset (1207) of adult compensation claimants with noise-induced hearing loss.
    • Statistical examination of relationships between conventional puretone thresholds and ARTs for tonal and broadband stimuli using SPSS.
    • Evaluation of ART dependency on puretone thresholds and predictive capabilities for audiogram features.

    • Average tonal ART remained constant up to 70 dB average loss, then increased rapidly.

    Conclusions:

    • The convergence principle for ARTs is valid up to 70 dB average loss in this population.
    • ART measurements are not sufficiently accurate for clinical use or medico-legal assessment of hearing loss in adults.
    • Predictive accuracy of ARTs is influenced by audiogram slope, which cannot be reliably predicted from ART measures.
