    Summary

Post-carotid endarterectomy, hypertension is common (66%). Intravenous hydralazine effectively lowered blood pressure in patients, proving safe for intraoperative hypertension management.

    Area of Science:

    • Vascular Surgery
    • Anesthesiology
    • Pharmacology

    Background:

    • Carotid endarterectomy is a surgical procedure to remove plaque from carotid arteries.
    • Hypertension is a common complication following this surgery.

    Purpose of the Study:

    • To determine the incidence of hypertension after carotid endarterectomy.
    • To evaluate the efficacy and safety of intravenous hydralazine for treating intraoperative hypertension.

    Main Methods:

    • Observational study of 39 patients (42 procedures).
    • Monitoring blood pressure changes.
    • Treatment of hypertension with intravenous hydralazine (20 +/- 8 mg).

    Main Results:

    • Hypertension occurred in 28 cases (66%).
    • Hydralazine administration resulted in significant blood pressure reduction: systolic by 46 +/- 22 mmHg, diastolic by 24 +/- 12 mmHg, and mean by 31 +/- 15 mmHg.
    • A minor increase in pulse rate (7 +/- 9 bpm) was observed.

    Conclusions:

    • Intraoperative hypertension is frequent after carotid endarterectomy.
    • Intravenous hydralazine is a safe and effective treatment for managing this hypertension.
