    Summary

    Melphalan, a chemotherapy drug, can cause interstitial lung disease in multiple myeloma patients. Stopping the drug and using corticosteroids improved lung imaging but gas exchange remained severely impaired.

    Background:

    Observation:

    • A patient undergoing melphalan therapy for multiple myeloma developed diffuse interstitial fibrosis.
    • The patient's lung condition was assessed through radiographic imaging and gas exchange measurements.

    Findings:

    • Cessation of melphalan administration and initiation of corticosteroid therapy led to progressive radiographic improvement.
    • Despite imaging improvements, the patient experienced persistent and severe impairment in gas exchange.

    Implications:

    • This case highlights a potential adverse pulmonary effect of melphalan therapy.
    • Early recognition and drug cessation may mitigate radiographic signs of lung fibrosis.
    • Severe gas exchange abnormalities may persist even after radiographic resolution, impacting patient prognosis.
