    Summary

    This study found that a noninvasive exercise test can detect differences in cardiac performance between healthy men and those with coronary artery disease (CAD). However, the test is not sensitive enough for diagnosing CAD in individual patients.

    Background:

    • Coronary artery disease (CAD) affects cardiac performance.
    • Noninvasive methods are needed to assess cardiac function during exercise.
    • Systolic time intervals (STI's) offer insights into cardiac mechanics.

    Purpose of the Study:

    • To evaluate a novel 3-stage cycle ergometer test.
    • To assess its ability to differentiate cardiac performance between healthy individuals and CAD patients.
    • To determine its utility in diagnosing CAD.

    Main Methods:

    • Administered a 3-stage cycle ergometer test to 15 healthy men and 20 men with confirmed CAD.
    • Measured systolic time intervals (STI's), stroke volume (SV), mean systolic ejection rate (MSER), and blood pressure noninvasively.

  • Main Results:

    • An inversely linear relationship was observed between STI's and heart rate (HR).
    • CAD patients (Group 2) exhibited higher QS2, LVET, PEP, and lower SV and MSER compared to healthy men (Group 1) across exercise HR's.
    • At a common HR of 100 bpm, QS2 and MSER were the most effective discriminators between groups.

    Conclusions:

    • The cycle ergometer test is sensitive enough to detect group differences in cardiac performance.
    • The test is not sufficiently sensitive for the individual diagnosis of coronary artery disease (CAD).
