    Summary

    Clinical assessment, pre-mastectomy biopsy, and mastectomy specimen examination all failed to accurately detect axillary lymph node metastasis in patients with Stage II disease. Accurate staging of axillary lymph node involvement remains a clinical challenge.

    Background:

    • Accurate axillary lymph node staging is crucial for breast cancer treatment.
    • Current methods for assessing axillary lymph node status have limitations.

    Purpose of the Study:

    • To compare the effectiveness of three methods in assessing axillary lymph node status.
    • To evaluate the diagnostic accuracy of clinical assessment, pre-mastectomy biopsy, and mastectomy specimen examination for lymph node metastasis.

    Main Methods:

    • Clinical assessment of axillary lymph nodes in 149 patients.
    • Histologic examination of axillary nodes biopsied prior to mastectomy in 54 patients.
    • Examination of lymph nodes in the axillary tail of 95 mastectomy specimens.

    Main Results:

    • Clinical assessment missed positive nodes in 45% of Stage II patients.
    • Pre-mastectomy biopsy missed metastatic disease in 42% of Stage II patients.
    • Mastectomy specimen examination missed axillary involvement in 14% of Stage II patients.

    Conclusions:

    • None of the evaluated methods accurately determined the extent of axillary lymph node involvement.
    • Improved diagnostic techniques are needed for precise axillary lymph node staging in breast cancer.
