Background:

• Struma ovarii is a rare germ cell tumor of the ovary containing mature thyroidal tissue.
• It can present as a benign or malignant entity, sometimes associated with hormonal dysfunction or ascites.

Observation:

• Two patients, aged 48 and 54, underwent surgery for presumed ovarian tumors.
• Histological examination confirmed the diagnosis of struma ovarii in both cases.
• One patient presented with ascites, a sign often associated with malignant ovarian tumors.

Findings:

• The struma ovarii in one patient behaved as a benign neoplasm.
• The second patient's struma ovarii was associated with ascites, mimicking a malignant ovarian tumor.
• Complete resolution of symptoms and disorders was observed in both patients following surgical intervention.

Implications:

• This case series highlights the importance of histological confirmation for struma ovarii diagnosis.
• It underscores that struma ovarii, even with malignant features like ascites, can have a benign clinical course post-resection.
• Surgical removal is effective in managing symptomatic struma ovarii, regardless of apparent malignant indicators.
