    Postpartum hematocrit monitoring is crucial. A single timed measurement within 24 hours postpartum is insufficient to detect the peak drop in most patients, necessitating a revised timing strategy.

    Background:

    • Postpartum hematocrit levels are monitored to assess blood loss after vaginal delivery.
    • Significant hematocrit drops can indicate postpartum hemorrhage or other complications.

    Purpose of the Study:

    • To evaluate the timing of peak hematocrit drop in the early postpartum period.
    • To determine the efficacy of single timed hematocrit measurements for detecting significant drops.

    Main Methods:

    • Prospective study of 23 patients undergoing vaginal delivery.
    • Hematocrit levels were measured at 8, 16, and 24 hours postpartum.
    • Patients with at least a 10% drop in hematocrit from baseline were analyzed.

    Main Results:

    • A significant hematocrit drop (≥10%) was observed in all 23 patients.
    • The peak hematocrit drop occurred at 8 hours in 4 patients, 16 hours in 9, and 24 hours in 10.
    • No single time point (8, 16, or 24 hours) accurately identified the peak drop in the majority of patients.

    Conclusions:

    • Standard postpartum hematocrit timing is inadequate for capturing peak drops in most women.
    • A revised, individualized timing strategy for postpartum hematocrit monitoring is proposed to improve detection of significant hemodilution.
