    Summary

    A woman with ulcerative colitis developed a large colon polyp. Pathologic examination revealed a unique lesion of filiform polyps trapping a fecal mass, not colon cancer.

    Background:

    • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
    • Colon polyps can be associated with increased risk of colorectal cancer.
    • Large polypoid lesions in UC patients warrant thorough investigation.

    Observation:

    • A 33-year-old female with a decade of ulcerative colitis presented with a large polypoid filling defect in the distal transverse colon.
    • Initial biopsy suggested an inflammatory polyp, but the radiographic appearance and patient history prompted subtotal colectomy.

    Findings:

    • Pathologic examination revealed an unusual aggregation of filiform polyps.
    • These filiform polyps had entrapped a semisolid fecal mass.
    • The lesion was not indicative of colorectal carcinoma.
