    Summary

    Transient confusion and asterixis were observed in two patients post-metrizamide lumbar myelography. These are the first documented instances of asterixis linked to this specific contrast agent.

    Background:

    • Lumbar myelography is an imaging procedure used to visualize the spinal canal.
    • Metrizamide is a non-ionic, water-soluble contrast agent commonly used in myelography.

    Observation:

    • Two patients developed transient confusion and asterixis (a tremor of the hands) after undergoing lumbar myelography.
    • Metrizamide was the contrast agent used in both cases.

    Findings:

    • Metabolic causes for the neurological symptoms were ruled out in both patients.
    • These represent the first reported cases of asterixis occurring subsequent to metrizamide lumbar myelography.

    Implications:

    • This suggests a potential, previously unrecognized adverse effect of metrizamide on neurological function.
    • Clinicians should be aware of the possibility of asterixis following metrizamide myelography.
    • Further research may be warranted to investigate the mechanism and incidence of this adverse event.
