Radical retropubic prostatectomy after transurethral prostatic resection

    Summary

Radical retropubic prostatectomy following transurethral resection of the prostate is a feasible procedure with acceptable morbidity. Most patients achieve good urinary control and survival rates are unaffected by prior prostate surgery.

    Area of Science:

    • Urology
    • Surgical Oncology

    Background:

    • Transurethral resection of the prostate (TURP) is a common procedure for benign prostatic hyperplasia.
    • Radical retropubic prostatectomy (RRP) is a standard treatment for localized prostate cancer.
    • The impact of prior TURP on the outcomes of subsequent RRP is not well-established.

    Purpose of the Study:

    • To evaluate the surgical difficulty, morbidity, and survival rates of radical retropubic prostatectomy in patients with a history of transurethral resection of the prostate.
    • To assess the functional outcomes, specifically urinary control, after RRP in this patient cohort.

    Main Methods:

    • Retrospective review of 36 patients who underwent RRP for adenocarcinoma after prior TURP.
    • Analysis of operative reports to assess surgical difficulty.

    Related Experiment Videos

  • Evaluation of postoperative morbidity and urinary continence.
  • Assessment of patient survival and follow-up duration.
  • Main Results:

    • The RRP procedure was routine in 67% of patients.
    • 50% of patients achieved perfect urinary control, while 4 patients (11%) were totally incontinent.
    • All patients remained alive, with 22% followed for 5 years or more.
    • No adverse effect on survival was observed due to prior TURP.

    Conclusions:

    • Radical retropubic prostatectomy can be performed safely and effectively in patients with a history of transurethral resection of the prostate.
    • Urinary continence rates are generally favorable, though a subset of patients may experience incontinence.
    • Prior TURP does not appear to negatively impact long-term survival following RRP for prostate cancer.
