    Background:

    • Persistent hyaloid vessels are congenital remnants of the embryonic hyaloid vasculature.
    • These vessels can occasionally lead to ocular complications, including hemorrhage.
    • Their association with persistent hyperplastic primary vitreous (PHPV) is noted.

    Observation:

    • A patient presented with bilateral persistent hyaloid vessels and experienced a prepapillary and intravitreal hemorrhage.
    • The hemorrhage led to clot hemolysis and retinal neovascularization, involving the fovea.

    Findings:

    • Despite the hemorrhage and retinal involvement, there was no vitreous separation.
    • Visual acuity remained 6/6-3 (20/20-3) at the time of examination.

    Implications:

    • Persistent hyaloid vessels, while uncommon causes of hemorrhage, are associated with persistent hyperplastic primary vitreous.
    • These vascular anomalies should be considered potential complicating factors during ocular surgeries.
