    Background:

    • Xanthomas are common manifestations of severe hyperlipidemia, typically presenting in skin and tendons.
    • Common types include xanthelasma, tuberous xanthoma, tendon xanthoma, and eruptive xanthoma.
    • Familial hypercholesterolemia is a genetic condition characterized by high cholesterol levels.

    Observation:

    • Two patients with hypercholesterolemia presented with xanthomas in unusual anatomical sites.
    • One patient had a xanthoma located deep within the mediastinum.
    • The second patient exhibited xanthomas within the muscles of the buttock.

    Findings:

    • These ectopic xanthomas presented as masses, initially simulating neoplastic tumors.
    • Pathological and chemical examinations confirmed the masses were typical xanthomata.
    • The findings highlight xanthomas occurring outside typical locations in hypercholesterolemia.

    Implications:

    • Ectopic xanthomas should be included in the differential diagnosis for unexplained masses in patients with familial hypercholesterolemia.
    • Recognition of these atypical presentations can prevent misdiagnosis and unnecessary aggressive treatment.
    • This expands the understanding of xanthoma distribution in severe lipid disorders.
