Cystic thymoma simulating pulmonary stenosis

A S Soorae, H M Stevenson

British Journal of Diseases of the Chest
    |April 1, 1980

    Summary

A cystic thymoma caused chest pain and a pulmonary murmur by compressing the pulmonary artery. Surgical removal of the thymoma resolved symptoms, highlighting treatment for this rare condition.

    Area of Science:

    • Cardiology
    • Thoracic Surgery
    • Pathology

    Background:

    • Thymic cysts are rare anterior mediastinal masses.
    • They can cause significant cardiovascular and pulmonary symptoms due to extrinsic compression.

    Observation:

    • A 36-year-old male presented with chest pain, an anterior mediastinal mass, and a loud pulmonary systolic murmur.
    • Intraoperative findings revealed a large, cystic thymoma compressing the right ventricular outflow tract and main pulmonary trunk.

    Findings:

    • Excision of the cystic thymoma led to the complete resolution of the systolic murmur, thrill, and chest pain.
    • The case illustrates a rare cause of acquired pulmonary stenosis due to extrinsic compression by a thymic cyst.

    Implications:

    • Highlights the importance of considering thymic cysts in the differential diagnosis of mediastinal masses presenting with cardiovascular symptoms.
    • Demonstrates the efficacy of surgical excision for symptomatic relief and management of thymoma-induced pulmonary artery compression.
