K Kanda, A Sari, K Nagai

    Critical Care Medicine
    |November 1, 1980
    PubMed
    Summary

A postpartum woman experienced sudden hypovolemic shock due to increased capillary permeability. Prompt treatment with plasma transfusions and norepinephrine led to a successful recovery and no recurrence within a year.

    Area of Science:

    • Obstetrics and Gynecology
    • Critical Care Medicine
    • Cardiovascular Physiology

    Background:

    • Postpartum complications can be severe and life-threatening.
    • Understanding rare causes of shock is crucial for timely intervention.
    • Capillary leak syndrome presents a diagnostic challenge.

    Observation:

    • A 29-year-old healthy woman developed sudden hypovolemic shock, anasarca, pleural effusion, and ascites postpartum.
    • Generalized increased capillary permeability was suspected as the cause of plasma leakage.

    Findings:

    • The patient was successfully treated with massive plasma transfusions, norepinephrine, and early respiratory support.
    • No recurrence of symptoms was observed during one year of follow-up.

    Implications:

    • This case highlights the importance of considering capillary leak syndrome in postpartum shock.
    • Aggressive fluid resuscitation and vasopressor support are vital.
    • Early respiratory support can improve outcomes in severe cases.
