    Background:

    • Viral hepatitis, specifically Hepatitis B virus (HBV) infection, is a known extrahepatic manifestation.
    • Myositis, an inflammatory condition of muscle, can occur in association with various infections.
    • The pathogenesis of viral hepatitis-associated myositis remains incompletely understood.

    Observation:

    • Electron microscopy identified osmiophilic deposits within the sarcolemma of striated muscle fibers.
    • These deposits were localized to the muscle tissue of a patient presenting with myositis secondary to viral hepatitis.
    • Immunohistochemical analysis revealed the presence of Hepatitis B surface antigen (HBsAg), Immunoglobulin G (IgG), and complement component 3 (C3) within these deposits.

    Findings:

    • The co-localization of HBsAg, IgG, and C3 in sarcolemmal osmiophilic deposits strongly suggests their composition as immune complexes.
    • These immune complexes are implicated as the primary causative agent of myositis in this patient.
    • The findings provide direct ultrastructural evidence linking HBV infection to immune-mediated muscle damage.

    Implications:

    • This study elucidates a potential mechanism for myositis development in viral hepatitis patients.
    • Understanding the role of immune complexes may lead to targeted therapeutic strategies for hepatitis-associated myositis.
    • Further research into immune complex deposition in viral infections could offer insights into other autoimmune phenomena.
